Job Description
Main duties and responsibilities:
– Monitoring the client’s health status and adjusting care plans as needed based on client needs
– Working with the family to ensure that they are comfortable with the care plan and know how to provide care if needed
– Scheduling appointments with physicians and other healthcare professionals to ensure proper treatment is being received
– Ensuring that the home environment is safe for the client by ordering new equipment or making repairs to items such as stairs or cabinets
– Coordinating with other service providers such as physical therapists, occupational therapists, and social workers
– Assessing the client’s needs and working with the family to develop a care plan that meets those needs
– Managing all aspects of the client’s care, such as hiring and training caregivers, helping with bathing and dressing, preparing meals, and administering medications
– Cultivating strong relationships with the client and his or her family in order to understand their needs and concerns
– Communicating regularly with case managers and other home care staff members about the client’s progress and needs
Required Experience in Handling:
– Must handle patients with peg tube, tracheostomy, colostomy and mechanical ventilator and CPAP Machine
– Child Care
– Newborn
– Advance Elderly care
Ability to commute/relocate:
